If you are a permanent and totally disabled veteran from a service-connected injury/disease and you are not receiving TRICARE as a retired member of the Armed Forces, your spouse may be entitled to CHAMPVA (Civilian Health and Medical Program of the Department of Veterans Affairs).

CHAMPVA is a health benefits program in which the Department of Veterans Affairs (VA) shares the cost of certain health care services and supplies with eligible beneficiaries. Medicare status has an impact on eligibility for CHAMPVA benefits. Individuals receiving Medicare must enroll in and remain enrolled in Medicare Part B.

To apply for the CHAMPVA benefit, contact CHAMPVA Eligibility at 800-733-8387 or visit www.va.gov/hac to request forms. You can also visit your local VA Regional Office or call 1-800-827-1000 for an application.
